Omni, in partnership with AdventHealth (Castle Rock and Parker Hospitals) and the Douglas County Health Department, led a Community Health Needs Assessment (CHNA) to identify and address critical health issues, equity gaps, and barriers to care in Douglas County. The team implemented a three-phased, mixed-methods approach designed to center community voices and stakeholder expertise. This process included a project kickoff and review of existing data, followed by key informant interviews and focus groups with underserved populations, culminating in an inclusive prioritization session with both community members and hospital leadership.
The final product was a clear, accessible CHNA report and action plan that both met IRS 501(r) requirements and supported strategic planning with actionable, community-informed recommendations. Omni’s role ensured that insights from across the county shaped every stage of the work, from data review to decision-making. This collaborative effort not only advanced health equity goals in the region but also laid the foundation for continued public health planning. In summer 2025, Omni will continue this work with the Douglas County Health Department through the development of a Public Health Improvement Plan, building on the momentum of the CHNA to guide long-term solutions.
